6/19/2023 EnchantaVaughn Approved Behavior: Jacaeri greeted QMHP-C by saying good evening as soon as she saw her. Mom was observed sitting quietly on the couch watching a movie with her other daughter. Jacaeri displayed productive behaviors and was very persistence throughout the day. Jacaeri appeared to be in an excited and upbeat mood as evidenced by him smiling and initiating a conversation with QMHP-C. Jacaeri expressed that he was feeling good and was going to have a good day. Behavior: Jacaeri transitioned into the next activity without conflict and proceeded to engage into meaningful conversation. Jacaeri was relieved to know that he wasn’t alone and that many boys his age go through the same things. Jacaeri attempted to distance himself from the session when he felt overwhelmed. Jacaeri was encouraged to take a break and try to relax. Jacaeri is aware that the sessions are for him to deescalate unwanted feelings and to challenge himself to overcome symptoms of ADHD. Jacaeri expressed positive thoughts. Intervention: QMHP-C verbally identified the Jacaeri's impulsiveness to the Jacaeri and encouraged the Jacaeri to use self-control strategies (i.e. decrease hyperactive behaviors. QMHP-C reviewed several strategies for conflict resolution and positive social behaviors with Jacaeri in efforts to increase Jacaeri’s awareness. The Jacaeri was encouraged to use good judgement, and respect authority figures. QMHP-C provided Jacaeri with a release of energy by deep breathing to help him relax and stay calm. QMHP-C provided simple praises to Jacaeri for his commitment to his progress. Intervention: QMHP-C modeled proper positive behavioral expectations for the Jacaeri to encourage additional layers of support at home. QMHP-C implemented individualized behavior management techniques with Jacaeri. QMHP-C processed with Jacaeri about psychoeducational activity while assisting with directions. QMHP-C explained the importance of the activity. QMHP-C provided feedback to Jacaeri in regard to questions asked. QMHP-C explored Jacaeri’s responses and provided appropriate feedback. QMHP-C supported Jacaeri making good decisions and having positive behaviors at home. Response: Jacaeri said he woke up feeling good and was ready to have a good day. Jacaeri expressed positive thoughts about himself. Jacaeri stated he didn't need help with anything at the moment but would say something if he needed help. Jacaeri communicated by nodding and exhibiting good eye contact. Jacaeri verbalized positive feelings about goals and objectives. Jacaeri communicated that he wants to work independently instead of as a family some days so that he could gain clarity of his personal feelings without taking it out on someone he loves. Response: Jacaeri displayed respectful, non-aggressive behaviors in the second half of session. Jacaeri avoided distractions in social setting, while utilizing anger management skills. Jacaeri seem to be heavily influenced by others and pressured into doing things he shouldn’t do. Jacaeri displays leadership skills by helping others and being good. Jacaeri acknowledged interventions and verbalized clear understanding of each intervention. Jacaeri engaged in reflective thinking and expressed his feelings to QMHP-C. Jacaeri communicated the do's and don'ts of home/school rules so that he shows more commitment to his choices.
